In addition to translation, an opening wedge effect is created to optimize correction of the 1–2 IMA. The distal fragment is then slightly plantarflexed relative to the proximal fragment, even leaving a slight gap on the plantar aspect of the osteotomy to be bone grafted with bone resected from the medial prominence that is created at the osteotomy site (Fig. 6). In our hands, this technique modification tends to avoid dorsiflexion of the distal fragment. We place a provisional K-wire from the proximal fragment across the osteotomy, crossing the first TMT joint if necessary to stabilize the osteotomy while keeping the provisional fixation out of the way of definitive fixation.
There have been multiple incisional approaches described for the Lapidus procedure. 12 The two-incisional approach uses the first tarsometatarsal joint incision as well as an incision over the medial first metatarsophalangeal joint. Finally, the single incision approach extends approximately 10 cm in length from the middle of the hallux proximal phalanx to the midportion of the medial cuneiform in a linear orientation medial and parallel to the extensor hallucis longus tendon. This is the approach preferred by the authors.
