In response to one question, people recording “financial hardship” in the previous 12 months, showed very high symptom levels. This group included many single parents, unemployed men and wives of unemployed men, so financial hardship recorded in this way certainly overlaps with several other factors. 2). Employed men had lower rates than others. Especially high were men unemployed and not seeking work, but numbers were small and they were mostly men with chronic sickness or disability. For unemployed men, but only if seeking work, higher scores were associated with longer periods of unemployment.
The search strategy ultimately adopted for this poorly defined and ill-standardised field was, therefore, necessarily broad. A wide range of searches were undertaken using EMBASE and MEDLINE international databases, in medical, psychological, sociological, and other journals, largely in English. Search terms included: • psychiatry, psychiatric symptoms/disorder/illness, mental disorder/ illness, emotional disorder/illness, mental health; • epidemiology, frequency, prevalence, incidence, community/population surveys; • social class/status, socio-economic class/status, income, wealth, material resources, education, employment, occupation; • inequality, inequity, disadvantage, poverty.
Standardised prevalence ratios (SPRs) followed standardised mortality ratios (SMRs) closely for these two regional groupings, though not all individual regions had exactly the same ranking. This north-south difference varied by occupational social class: social classes I–II showed none, but the greatest difference was in social classes IIINM–IIIM, that is, greater than social classes IV–V. However, when adjusted for several factors known to be associated with psychiatric morbidity, there was no statistical significance in the north–south difference.
