These machines produce a puff of air that impinges on the corneal surface to cause applanation. The time interval between the start of the applanation force and the appearance of a flattened cornea is taken as the measure of IOP.
Most Schi0tz readings are within 2 to 5 mmHg of true IOP. Only rarely does a diagnosis need a more accurate measurement of IOP in clinical practice. The Goldmann applanation tonometer appears to be more accurate than the Schi0tz tonometer. The range of replicate measurements is no more than 2 to 3 mmHg with this tonometer. Problems that may be encountered with use of the Goldmann tonometer are the effects of corneal scars and edema or severe astigmatism. Evidence from studies of animal eyes suggests that corneal properties do affect applanation readings of IOP, but the Goldmann tonometer is a highly accurate device when restricted to human eyes with normal corneas.
